The report on the fire determined that unprotected vertical and horizontal penetrations provided one of the major contributing causes of the rapid, erratic spread of smoke and fire. For that reason, building codes began to address the mechanical and electrical openings in floors, ceiling and walls to make sure that an uninterrupted fire barrier was installed to maintain the integrity of fire-resistance ratings throughout buildings.
